Cost: $50 Deposit (Refundable)

Refundable at handler's discretion if they:

• Complete all participant waivers

• Complete PT test

• Attend all training sessions

*Application fee will be collected after completing this form. If you are not selected for the course, your application fee will be refunded to you. Upon selection and completion of requirements, handler has the option to receive full or partial refund or donate the entire amount to Spike's K9 Fund.

Release & Agreement

I read and understand the terms regarding the Spike's School fitness test. I understand that if I do not pass the test, I will be personally responsible for Spike's School fees and will not be refunded the $50 application fee.

This application must be completed two months prior to the workshop you are applying to receive sponsorship for. Incomplete applications will be discarded without consideration. For questions about Spike's School or CFTE workshops, contact deana@thecfte.com.
